If your child has an autism spectrum disorder diagnosis within 2 years, a referral is to be faxed to
817-494-0099. When that fax is received a representative will call you to begin the intake process.
When all intake paperwork has been completed and insurance has authorized the clinic to assess your child the representative will set up an appointment.
